Once the form has been filled out, bring it to the appropriate office in the district - elementary, middle or high school. At the school, parents will be asked to sign a release form, and to provide proof of residency.

To provide proof of residency please bring one or more of the following:

• current drivers license with 911 address on it
• voter registration
• recent phone or electric bill in the name of the head-of-household
• letter from employer confirming residence in district
• copy of purchase contract for home (price can be blacked out)
• copy of letter from attorney confirming date and time of closing on property

Note: Forms may not be mailed to the district, nor may registration take place over the phone. All parents must come to the school when registering their child(ren).
