Salmonella Infection, Immunization, and Lead Poisoning
Physicals are scheduled for May 6th, 20th, & June 6th, 2009
You may provide a copy of your child's
physical done by their primary care physician.
Physicals are required upon entry to school, and grades
Pre-K, K, 2nd, & 4th.
Health Screenings are required for all grade level .Vision
and hearing are completed in K, 1st, & 3rd grade. Vision is
completed in 2nd grade with hearing as needed. For 4th grade
vision & hearing are completed on an as needed basis.
